STATEMENT OF WORK:
Develop monthly calendar of events; design and implement the daily schedule; ensure that all planned activities are implemented.  Analyze the activity needs of the participants and develop programs to meet those needs. Complete recreation assessments, reassessments, and in conjunction with participant, develop recreation outcomes for care plan; actively participate in care plan conferences.  Responsible for ensuring that care plan outcomes are achieved and noted in participant file. Responsible for all participant Activity Flow Sheets being completed on a daily basis.  Observe and report to Site Supervisor/Center Director or Social Worker significant changes in participant's behavior.  Maintain activity area at all times.  Performs other job related duties deemed necessary and/or assigned by the Site Supervisor/Center Director.
 
ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Develops and submits, before the end of the calendar month, an imaginative and varied monthly calendar of events. Activities for participants are designed to meet personal interest, promote growth, enhance self-image, and improve or maintain functional levels when possible.  Activities encompass the participant's individual needs in the areas of education, socialization, recreation, physical, and mental development.  Attention is given to participant's individual ethnic, cultural, and religious beliefs. Emphasis is placed on scheduled times for activities as well as time for rest, relaxation, and attention to personal needs.
  • Implements daily schedules of activities.  Leads and encourages participants to join in groups facilitated by staff or volunteer.  Special care is to be given to promote participants highest level of functioning.
  • Actively participates in care plan conferences.  Includes completing recreation assessments, reassessments, and developing care plan outcomes; following through on expected outcomes and documenting progress toward outcomes.
  • Supervises all recreation leaders and assistants as directed by Site Supervisor/Center Director.  Ensures that all planned activities are implemented.  Provides training and guidance to recreation staff in activity development and facilitation.
  • Coordinates and supervises participant outings and special events.
  • Observes participant behavior; report any physical or behavioral changes to Site Supervisor/Center Director or Social Worker.
  • Writes progress notes as required by Site Supervisor/Center Director.  Ensures that progress notes are reflective of the participant's care plan.  Preserves confidentiality of all records.
  • Completes both major and minor activity purchases.  Ensures that enough materials are on hand so that programs can be carried out as planned minimizing the need for last minute purchases.  May be responsible for petty cash.
  • Maintains activity area.  Ensures that all activity areas are clean, neat, orderly, and that activity supplies are available for activity.  Responsible for the locked storage of paints, glue, scissors, and other potentially hazardous materials.  Decorates activity space and bulletin boards for the appropriate season.
  • Displays enthusiasm and maintain a positive attitude in working with the participants.
  • Implements licensure and accreditation standards related to the recreation program.
  • Monitors the safety and security of participants at all times.
  • Participates in ongoing training, including training sessions offered at the Foundation's administrative offices.
  • Assists with meals according to CACFP guidelines.
  • May be required to evaluate, counsel, or discipline recreation staff under the guidance of the Site Supervisor/Center Director.
  • May be asked to participate in fundraising activities.
  • Other duties as needed.
 
KNOWLEDGE, SKILLS AND ABILITIES:
        Minimum Required:
  • Must be 21 years of age or older.
  • Knowledge of planning, developing, and implementing activity programs.
  • Must be alert to the needs of the participants; have the ability to work under conditions of frequent interruptions; and to work on several projects at the same time.
  • Ability to work with challenging behaviors expressed by participants, i.e. verbal and physical aggression, wandering, etc.
  • Accuracy in documenting observations of activity performance.
  • High energy output during activity program.
  • Knowledge of/or experience in the gathering of data, evaluation of data, and external dissemination of data as part of the ongoing performance improvement process.
  • Must have valid Arizona driver’s license and vehicle insurance.
  • Must be insurable by Foundation for Senior Living when driving FSL vehicles.
  • Ability to become CPR/First Aid certified.
  • Must have, or be able to obtain, Level One Fingerprint Clearance Card through Department of Public Safety’s background check/fingerprint clearance process.
  • Ability to obtain Food Handlers card. May be requested to assist in the kitchen.  May be requested to assist with the serving of meal and snacks.
  • Must not have any restrictions for physical work for which reasonable accommodation cannot be made.
 
        Preferred:
 
  • Previous Adult Day Health Services experience.
 
WORK EXPERIENCE AND EDUCATION:
        Minimum Required:
  • A B.F.A., B.A. or B.S. degree in therapy, art, music or related field or certification as an Activity Coordinator through National Activity Association of Professionals; three or more years of experience administering an activity program may substitute for a degree.
       
Preferred:
 
  • Experience or training in working with older adults, adults with disabilities, or adults with SMI.
  • Experience in developing and publishing a calendar of monthly activities.
 
 
 
WORKING CONDITIONS AND PHYSICAL DEMANDS:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position. 
  • Works in a clean, well-lit, air conditioned or heated office.
  • Physically active most of the day coordinating one or more activity programs.
  • Must be able to lift up to 25 pounds.
  • Required to assist patients with transfers.
  • Required to assist participants with the use of the bathroom.
  • May be requested to operate a FSL vehicle for community outings
  • May be requested to assist participants in entering and exiting the van.
  • Exposure to potentially infectious fluids such as blood or body fluids (medium to high).
  • Exposure to latex, plastic, and/or materials which are used for personal protective equipment.
  • Demonstrates and is proficient at safety practices on the job.
