Full Time with 8 or 12 hour shift option + Rotating On-call every 5 weeks
Area Coverage : Central & West Valley


Plans and provides direct and supervisory intermittent skilled clients care in the home under the direction of the Clinical Administrator.  Provides professional nursing services for program clients, training to house staff and high quality programs in each home.  Assesses client needs to plan, implement, coordinate and evaluate therapeutic services.  Performs any other job-related tasks deemed necessary and/or assigned by Supervisor.  Area Supervisor oversees all medication and nursing processes.
  • Utilizes and documents nursing process to serve clients and their families, as follows:
  • Assess, identify and document present and potential physical and emotional needs of the clients as they relate to nursing intervention.
  • Develop nursing care plan to meet identified client needs in cooperation with the client and under the direction of the treating medical provider.  Discuss plans with house staff, the clinical team members.
  • Provide skilled nursing care and instruction to clients and instruct staff in providing care as necessary.
  • Assist clients with medications and oversees medication monitoring.  Oversee staff compliance with AGL medication policy and procedure.  Complete quarterly medication reviews.  Update other staff on medication orders and other medical provider orders in the client record.
  • Record all new medication orders and other medical provider orders in the client record.
  • Educate staff and clients regarding medication effects.
  • Become familiar with all client medications and their side effects.  Monitor for side effects.
  • Assist client with ROM exercises and other therapeutic activities
  • Report to the medical provider or dentist any unusual changes in the client’s condition and reviews client medications.
  • Monitor staff compliance with nursing care plans.
  • Conduct assessments of client health status.
  • Identify the need for and request professional consultation from Clinical Management as Client needs dictate.
  • Coordinate discharge planning with the Clinical Program Manager/designee.
  • Visit and monitor AGL homes on a regular schedule and as needed to evaluate the needs of clients and the general quality of service.
  • Conduct in-service training.
  • Participate in management staff meetings.
  • Assist in problem solving with Team Supervisors, ILAs and other staff members when required.
  • Attend client staffing’s  and participates as a member of the service planning team.
  • Prepare reports in an timely manner
  • Act as a role model in maintaining FSL-AGL standards in accordance with agency policies and procedures and service standards.
  • Provide on-call nursing coverage, as required on a rotating basis with other nursing staff.
  • Must have completion of Crisis Intervention training prior to using an emergency safety response (R-10-716.E.)
  • Participate in the Performance and Quality Improvement initiative, offering input, suggesting performance improvements, and performing other PQI duties as requested.
  • Other duties as assigned
        Minimum Required
  • Knowledge of needs of elderly, SMI clients, home health needs, community resources, role of professional nurse, ability to apply nursing process to agency clients, ability to coordinate services.
  • Ability to think independently, empathy for aging clients, and understanding of mental illness.
  • Personal qualities include: flexibility, tolerance of varied lifestyles and personal behaviors, sound judgment, reliability.
  • Must not have any restrictions for physical work for which reasonable accommodation cannot be made. 
        Minimum Required
  • Licensed Registered Nurse by the Arizona State Board of Nursing
  • Current Arizona driver’s license and insurance
  • Current personal professional liability insurance, Company paid
  • Must be able to perform work both inside and outside, which requires stooping, bending and lifting of approximately 50 pounds.
  • Must be able to obtain Level I Fingerprint Clearance Card through a background and fingerprint clearance through the Department of Public Safety.
  • 2 years behavioral health experience.
  • Minimum one year community health experience and/or psychiatric  nursing.
  • Membership in nursing and/or community health organization.
