DescriptionSTATEMENT OF WORK:
Administers and assists with adult day health services (ADHS)/Outpatient Clinic, Medication Services under the licensure of the Arizona Department of Health Services, Offices of Behavioral Health and Assisted Living Licensing. Responsible for coordinating the program including: intake and discharge of participants; documentation, monthly reports; staff hiring, training, supervising, evaluating and termination; clinical supervision of staff; marketing of program; works with the Advisory Council or Board of Directors; cooperates with community agencies and other external stakeholders; coordinates the maintenance of building and equipment. Performs other job-related tasks deemed necessary and/or assigned by the Program Director.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Responsible for daily supervision of ADHS/Outpatient Clinic Medication Services participants who require care and supervision by ensuring that required number of personnel are on duty; develops written staffing schedule to assure participant supervision at all times.
- Develops staff skills for working with the frail elderly, disabled adult and/or participant with SMI; sends staff to training sessions needed for their positions.
- Coordinates team efforts through regular team meetings. Assures that quality care and supervision are appropriate to participant needs.
- Responsible for final approval of participant intake and discharge. Oversees assessments of participant. Oversees regular care planning conferences to determine participant’s progress toward outcomes.
- Coordinates recreation, socialization, health, transportation, and social service components of ADHS/Outpatient Clinic Medication Services program. Ensures that monthly calendar of events is posted and activities are appropriate to participant’s level, functioning, and interest. Ensures that weekly menus are posted and have been approved for nutritional value; meals are prepared and served to comply with government regulations. All participants’ physical and mental health needs are maintained by qualified health personnel. Ensures that safe and coordinates transportation is available to all participants either through the Foundation’s fleet or through public/private transportation. Provides information and referral services to all participants and caregivers.
- Hires, trains, supervises, evaluates and terminates staff while complying with FSL Programs personnel policies and procedures. Maintains personnel files for all staff including: accurate job descriptions, current certifications, timely evaluations, chronologicals, training, and clinical supervision. Monitors employee hours and signs time sheets. Authorizes mileage reimbursements, maintenance, and petty cash.
- Prepares basis for and monitors budgets. Works with FSL Programs administration in order to maintain zero deficit position ensuring that expenses do not exceed actual income level. Explores and encourages other sources of funding in order to promote program financial stability.
- Works with site staff in marketing the program(s), according to FSL marketing plan and marketing team direction to ensure census stability and to ensure budgetary needs are adequately met.
- Ensures compliance with CACFP programs along with other funding, such as, but not limited to DDD, VA, ALTCS, SAIL, United Way and CDBG. Promotes Adult Day Health Services/Outpatient Clinic, Medication Services program in the community.
- Complies with government contract regulations and provides monthly, quarterly, statistical and financial reports.
- Responsible for collection and dissemination of performance indicators as related to outcome measures.
- Responsible for maintaining licensure requirements under DHS and BHS. As a member of the Foundation ADHS leadership team, is responsible for preparing and maintaining standards of accreditation through CARF.
- Oversees safety committee to ensure that all fire, disaster, and civil disaster drills are completed and documented. Ensures that all staff are trained in Universal Precautions and that proper infection controls are maintained at all times.
- Supports and encourages linkage with other FSL programs, services and facilities. Complies with FSL Programs policies and procedures.
- Works with Advisory Council or Board of Directors by supplying program information, scheduling and attending meetings. Prepares reports as requested by the Program Director so that the program is fully understood within FSL Programs and the community by dissemination of correct and consistent information.
- Works with community agencies; makes conscious effort to establish rapport, work relationships, and contacts with broad range of people and officials, such as City Council, Mayor, United Way, and other service agencies. Cooperates with community agencies by researching and utilizing available services, sitting on committees, and assisted in workshops and seminars of other agencies when invited.
- May be required to coordinate a fundraiser project to support the program.
- May share on-call coverage for the division with the Program Director.
- Other duties as needed.
KNOWLEDGE, SKILLS AND ABILITIES:
- Must be 21 years of age or older.
- Knowledge of and ability to utilize effective management and leadership techniques.
- Knowledge of the special needs of the participant populations.
- Understanding and ability to facilitate an interdisciplinary approach for providing quality service to center participants.
- Must have ability to comprehend, retain, and follow regulations and procedures relative to the provision of ADHS/Outpatient Clinic, Medication Services.
- Ability to develop and follow and center’s operating budget.
- Ability to provide solid leadership and direction in life or death situations.
- Considerable initiative and ingenuity involved in the administration and operation of a program offering transportation, recreation, social services and health services to high-risk participants and their families.
- Knowledge of, or experience in, the gathering of data, evaluation of data, analysis of data and external dissemination of data as a part of the ongoing performance improvement process.
- Must be able to model and work with and train staff in the management of challenging behaviors expressed by participants, i.e., wandering, verbal and physical aggression, etc.
- Must have valid Arizona driver’s license and vehicle insurance.
- Must be insurable by Foundation for Senior Living when driving FSL vehicles.
- Must be able to obtain CPR/First Aid certification.
- Must have, or be able to obtain, a Level One Fingerprint Clearance Card through the Department of Public Safety background check/fingerprint clearance process.
- Must be able to obtain a Food Handlers Card; may be requested to obtain a Food Manager Certification; may be requested to assist in the kitchen.
- Must not have any restrictions for physical work for which reasonable accommodation cannot be made.
- Leadership skills necessary to mentor others in ADHS.
WORK EXPERIENCE AND EDUCATION:
- Bachelor’s degree in health-related, social service, recreation, administration of behavioral science, or applicable licensed health care professional.
- At least three years’ paid experience in human services, including one year of supervisory experience.
- Experience working with older adults, adults with physical disabilities, and adults experiencing mental health issues.
- Experience in the administrative supervision of adult day services.
- Gerontology courses helpful.

- Works in a clean, well-lit, air conditioned or heated office.
- Minimal noise from office machines. Has own desk and use of telephone in office.
- May spend portion of the day sitting at desk. Also moves about facility and grounds.
- While performing the duties of this position, the employee is regularly required to: talk and hear, including over the telephone, in normal range; use hands to finger, handle or feel objects, tools/equipment or controls; sit for extended periods of time, stand, reach with hands and arms, and walk. Occasionally required to stoop, kneel or crouch; lift and/or move up to 25 pounds. Vision required includes close vision, distance vision, and ability to adjust focus.
- May be requested to assist participants in transferring from wheelchair to chair.
- May be requested to operate FSL vehicle.
- May be requested to assist participants with the use of the bathroom.
- Exposure to potentially infectious fluids, such as blood or body fluids (low).
- Exposure to latex, plastic, and/or materials that are used for personal protective equipment.
- Must be able to lift up to 25 pounds.
- Demonstrates and is proficient at safety practices on the job.
