Description
EXCITING TEMPORARY OPPORTUNITY WITH A FOCUS TO HELP VULNERABLE ADULTS! 
 

Under the direct supervision of the Care by Design (CBD) Program Director.  Responsible for full caseload, screening and assessing clients for appropriate services and resources, home assessments, and community outreach and collaboration. Responsible for services which include telephone consultations, screenings for program eligibility, completing utility, appliance, and/or financial assistance applications, resource identification, and information and referral.  

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conducts weekly home and in-office visits in Maricopa County
  • Provides assistance to community members using virtual technology when necessary
  • Assists community members in need with resources and application assistance
  • Completes documentation in Care by Design database
  • Performs client-centered and solution-focused services to help client achieve wellness and autonomy
  • Initiates internal and external referrals
  • Coordinates and provides care in a safe, timely, effective, efficient, and client-centered manner
  • Reviews client progress and assists with intervention completion to ensure satisfaction
  • Develops and maintains effective communication with all team members, clients, and staff
  • Other duties as assigned by the Care by Design Program Director

 

Varied:

  • Prepares monthly statistics/reports for director
  • Completes assessments with walk-in clients
  • Assists with administrative tasks as needed
  • Participates in routine staff meetings and care conferences as needed
  • Cross-trains in other CBD staff roles
  • Community Outreach as needed

 

KNOWLEDGE, SKILLS AND ABILITIES:

        Minimum Required:

  • Experience in assessment, intervention planning, service implementation, client progress monitoring, and outcome evaluation
  • Knowledge of community resources
  • Ability to problem solve, multitask, and follow directions
  • Demonstrates high level of competency as it relates to using cell phones and general navigation of computer software programs
  • Arizona Driver’s license and personal vehicle
  • Must demonstrate good organization skills and be detail-oriented
  • Must be self-motivated and use good judgement to resolve client issues

 

WORK EXPERIENCE AND EDUCATION:

        Minimum Required:

  • Bachelor’s Degree

 

        Preferred:

  • Bachelor’s Degree in Psychology, Social Work, or Human Services
  • At least 3 years professional case management experience with senior population
  • Bilingual-English/Spanish
