Under the oversight of the Program Manager, this position is responsible for the hiring, scheduling, orientation,supervision, monitoring and evaluation of four to ten Independent Living Aides (ILAs) in an Assisted Group Living (AGL) home.  The Team Supervisor will maintain a complete daily coverage schedule (24 hours) for the home.  Team Supervisor will develop and maintain backup and emergency coverage plans for the home in coordination with the Program Manager.  Other responsibilities include, participation in developing and executing Individual Client Service Plans, which may include provision of support, coordination with case management agencies, monitoring service provision and advocacy for needed services.  Team Supervisor will be responsible for the  management of the home budget, control of purchase cards and petty cash funds for home operations, meal planning,  development & implementation of activity schedules, coordination of client appointments for services outside the home, home cleanliness, home maintenance, attendance at required training, inservice training and other duties as assigned.  Team supervisors will work  with their staff in providing direct care and  also  provide supervisory functions. This position will be on-call to home staff at times specified during the work week and may share on-call responsibilities in rotation with other Team Supervisors.
(Monday- Friday 8am-5pm +On Call) 
  • Problem solve with clients and team members concerning daily care and service needs.
  • Advocate for needed client services.
  • Work cooperatively with clinical team to develop and implement the ISP.
  • Serve as a Liaison with clients and clinical team.
  • Coordinate care with off-site programs and activities as necessary.
  • Provide support, encouragement and assistance to ILAs in the performance of their duties.  Provide direct care.
  • With assistance from the other staff, hire and maintain the authorized staff level for one AGL home.
  • Assist in the development of and conduct required initial orientation and training for all direct care staff.
  • Participate in management staff meetings.
  • Meet annual requirements for continuing educations and training.
  • Assist staff in meeting their required annual educational training.
  • Assist in problem solving with other Team Supervisors, ILAs and staff members when client care needs, schedules or staff require it.
  • Identify the need for and request professional consultation from Program Manager as client and/or staff needs dictate.
  • Monitor and assure compliance with AGL clinical and administrative documentation requirements and other AGL policy and procedure.
  • Participate in overall AGL policy development,strategic planning and management decision making.
  • Prepare reports in timely manner.
  • Accurately communicate the needs of staff, clients and management to and from staff and management.
  • Provide on-call supervisory coverage on a rotating basis with other management staff.
  • Maintain accurate staff time records.
  • Responsible for management of a petty cash account and company purchasing card.   Prepare current, accurate and complete reports of petty cash/purchasing card expenses as required by AGL Policy and Procedure.
  • Meet with the Clinical Program Manager or designee on a regular basis for clinical supervision.  Follow up on written and oral directions concerning client care.
  • Transport clients to regular and unscheduled medical or psychiatric appointments and conduct follow up to ensure changes in orders are communicated to appropriate clinical staff.
  • Informs the Clinical Program Manager or Program Manager of changes in client condition, medical orders and other contacts as required by AGL Policy and Procedure.
  • Participate in the Performance and Quality Improvement initiative at the agency and program level by learning about the initiative, offering input, suggesting performance improvements, and performing other PQI duties as requested.
  • Must have completion of Crisis Intervention training prior to using an emergency safety response.
  • Perform counseling functions with clients based upon experience and education as assigned.
  • Other duties as required and/or assigned.

        Minimum Required
  • Ability to communicate information necessary for quality program operations across all levels, including clients both orally and in writing.
  • Basic computer skills (i.e., Microsoft Office:  Word, Excel, Outlook, etc.)
  • Must be able to complete pre-employment exam with at least a  70% score and satisfactorily complete the job specific online assessment. 
  • Must be able to accurately document client progress in the electronic medical record, according to policy and standards.
  • Solid understanding of behavioral health diagnoses and symptoms.
  • Personal qualities including: flexibility, tolerance of varied cultures and behavior, sound judgment, good health, reliability, honesty, personal sensitivity, warmth and caring in relationships.
  • Must be able to work independently as well as develop and supervise cohesive teams.
  • Must have, or be able to obtain, Level 1 Fingerprint Clearance Card through a background and fingerprint clearance through the Department of Public Safety.
  • Must not have any restrictions for physical work for which reasonable accommodation cannot be made.
  • Must be able to de-escalate volatile behavioral situations as they arise.

        Minimum Required
  • High School Diploma or GED required.
  • 21 years or older.
  • Valid AZ Driver's License and insurance, and available transportation within the community.
  • Ability to drive FSL vehicles and maintain a valid driver’s license and be insurable by FSL.
  • Two years experience in behavioral field and one year supervisory experience; combination of training, experience and/or education may substitute.
  • Must be able to obtain Certified Caregiver Certification    
  • Some college coursework.
  • Five years’ experience in the behavioral health field and three years’ supervisory experience is desirable
  • Experience with SMI clients living in the community setting
